PCCOE Catalyst Grant Program Overview
The PCCOE Catalyst Grant Program supports innovation, fosters collaboration, and funds education to improve patient care and patient experience for those experiencing serious illness. Aligning with the PCCOE mission to advance palliative care through research, education, and clinical care, the Catalyst Grant program serves to broaden the reach and deepen the impact of both palliative care and serious illness care at Stanford Healthcare.
Definitions
Palliative Care: Specialized medical care focusing on quality of life and value-based communication delivered by a multidisciplinary team in an acute care or specialty clinic setting.
PalliHealth: Stanford’s unique serious illness care program. An innovative network of clinical programming that seeks to improve patient care and patient experience by using the transdisciplinary principles of palliative care to enhance quality of life throughout the course of serious illness across all settings of the healthcare system.
Eligibility:
This call for applications is for projects focused on palliative care or improving serious illness at Stanford Health Care.
We are specifically looking for applications from interdisciplinary teams; at least one team member must be on the faculty of the Palliative Care Section and the School of Medicine.
Fellows, clinician educators, UML and UTL faculty are all encouraged to apply. Fellows and med scholars may apply with a faculty mentor as PI.
Requirements:
Applicants are required to provide:
Project Title: Provide the official title of your project.
Investigator Information: List the full name and email of the PI and any team members.
Background, Aims, and Approach(Maximum 2 pages)
Background and Significance: Describe the background, relevance to palliative or serious illness care, and the knowledge gap your project addresses.
Specific Aims: Outline 1-3 specific aims with associated hypotheses, activities, and expected outcomes. Ensure the scope is feasible within 1-2 years.
Approach: Detail the methods and strategies you will use to achieve your aims.
Supplemental Materials(Not included in the page count)
Timeline: Provide a detailed timeline highlighting project milestones and expected completion dates.
Budget Justification: Complete a budget form and provide a brief rationale for all resource allocations and cost estimates.
Contribution to PalliHealth: In 2-3 sentences, explain how this project will support the growth and expansion of Stanford’s PalliHealth program to improve care across settings and disciplines.
Research Compliance: Indicate if IRB approval is needed and specify the project location(s).
Operational Partner Letter: Include a letter from a Stanford Health Care operational partner, indicating their support and participation in the project.
Additional Documents: Attach any relevant documents (e.g., study protocols, surveys) that help clarify your project.
Review Criteria
Significance and Innovation: Does the project introduce novel and creative approaches, or does it build upon or adapt trusted interventions? Does it include small tests of change or proof of concepts that can demonstrate feasibility or scalability in palliative or serious illness care?
Impact: What is the potential for the project to broadly impact learners, patients, or clinical care across Stanford Health Care?
Feasibility: Is the project timeline realistic? Can the proposed activities be completed within 1-2 years?
Alignment & Contribution to PalliHealth: How does the project directly support the growth of the PalliHealth program, and how will it enhance patient care or experience?
